What is profit margin?

Quick answer

Profit margin is the share of a clinic's collections left as profit after paying all costs, including salaries, rent, consumables, marketing and the doctor's own fair salary.

What it means

Profit margin shows how much of every rupee collected stays in the practice once the bills are paid. Two clinics with the same collections can have very different margins, and the one with the better margin is the stronger business.

Many doctors look at collections and assume the practice is doing well, then wonder why there is little left at the end of the year. Profit margin is the reality check. It also matters when you want to expand, bring in a partner or sell, because buyers and lenders look at profit, not revenue.

Pay yourself a fair salary for your clinical work before you calculate it. Otherwise the margin looks better than the business really is.

How to calculate it

Profit margin = (collections minus total costs) ÷ collections × 100

Total costs include salaries, rent, consumables, equipment payments, marketing, software and a fair salary for the doctor's clinical time.

For example, a clinic collects ₹20,00,000 in a month and its total costs are ₹9,00,000. Profit = ₹11,00,000. Profit margin = ₹11,00,000 ÷ ₹20,00,000 × 100 = 55%.

What good looks like

HBA's benchmark is a profit margin of around 50 to 60%, with the books for each month closed by the seventh of the following month. Late books mean late decisions. Our pricing and profitability work starts with exactly this number.

How to improve it

  • Close your books by the seventh of every month so you see problems while you can still act.
  • Review prices yearly. Many clinics have not changed fees in years while every cost has risen.
  • Cut marketing spend that does not bring treated patients, and fix conversion before you add more.
  • Check consumable use and supplier rates for your highest-volume treatments.
  • Review staffing against the diary, so you have the right team at peak hours rather than more people all day.

Frequently asked questions

What is a good profit margin for a clinic in India?

HBA's benchmark is around 50 to 60% of collections, after paying all costs including a fair salary for the doctor's clinical work. If your margin is lower, check pricing, unproductive marketing spend, consumables and staffing. Close your books by the seventh of each month so you can act quickly.

Should I include my own salary when calculating clinic profit?

Yes. Pay yourself a fair salary for the clinical work you do, then calculate profit. If you skip it, the practice looks more profitable than it is, and you cannot tell whether the business would survive with another doctor in your chair. It also matters for valuation.
